We are celebrating completing a Continuous Quality Improvement Project !
Recently we celebrated completion of a Continuous Quality Improvement project in which Māori and Pasifika patients with pre-diabetes were supported to prevent progression to Type 2 diabetes through early intervention, education, and holistic, culturally centred care.
Participant feedback was overwhelmingly positive, and outcomes included improvements in health literacy, medication adherence and clinical measurements such as blood pressure, cholesterol and HbA1c.
Plans are underway to build this model of care into the practice. We are very proud of this achievement and congratulate the team who drove this project.
