Health Improvement Practitioner (HIP)

Our HIP’s are employed by Nelson Bays Primary Health, they are at Stoke Medical five part days a week. Health Improvement Practitioners (HIPs) are behavioural health clinicians.

HIPs help with any issues impacting on your health and wellbeing. HIPs deliver focused 20-30 minute brief intervention for all age groups.

Services are free and you can often be seen on the same day.

HIPs can help with:

- Managing thoughts, feelings, and behaviours

- Stress, anxiety, or feeling down

- Coping with Grief and Loss

- Sleep

- Alcohol and Drug use

- Relationship issues

- Coping with chronic health conditions

- Managing high cholesterol, diabetes, or cardiac conditions

- Accessing other services


DEBBIE SUMNER

Health Coach

Kia Ora everyone, my name is Debbie Sumner and I am the Health coach at Stoke Medical. I work here on a Monday morning and on a Wednesday and Friday. I offer free support to support adults 18+ here in the practice and out in the community. I can help support you to identify the things you would like to work on, help with goal setting, making lifestyle change's and I am also a friendly face to check in with.

This service can help you with

-Living well with long-term conditions including diabetes, high cholesterol and blood pressure.

-Managing stress

-Support with developing healthier lifestyle habits such as eating well and being active.

-Connecting to other community supports like quitting smoking, housing and food supports.

-Engaging with community activities.

In my spare time, I am also a yoga teacher, teaching classes in the community and at special events. I enjoy camping, the outdoors, beach walks, reading books and spending time with my husband and two children.

Feel free to chat with the reception team to make a booking.

MERE WETERE

Health Coach

Mere works Monday afternoons, all day Tuesdays and Thursdays.

She supports adults 18+ in their practice and community.

She will work with you to identify the things you would like to work on, help with goal setting, making changes, and are a friendly face to check-in with. Generally appts with Health Coaches can be between 30-60 minutes and they also have the flexibility to offer home visits.

This service can support you with:

• Emotional well-being and stress management

• Healthy Lifestyle & sleep

• Exercise & being active

• Healthy eating

• Whānau /family & relationship support

• Quitting Smoking/Vaping

• Living well with long term conditions including Diabetes, High Cholesterol and Blood Pressure

• Connecting to other services who can help with things, like a kaupapa Maori health service, quitting smoking, housing, food supports or social connection.

We work on all aspects of wellbeing using the Te Whare Tapa Wha Model - Hinengaro (thinking), Wairua (Spirituality/Mana), Tinana (Body), Whanaungatanga (Family/Connection), in a culturally safe way.

We may also work on projects that target Maori, Pasifika or Asian whanau due to their health outcomes with regard to certain conditions such as diabetes.

This is a free service to patients.


TONIA TALBOT

Dietician

Tonia is employed by Nelson Bays Primary Health.  She attends Stoke Medical once a fortnight and is happy to see patients for diet and nutritional advice.  To make an appointment with the Dietician phone Stoke & Waimea Medical on 547 7488.
